1. I have been informed of the nature of the training and activities my child will participate in.

2. I understand that there are inherent risks associated with physical activities, and I release Foundations 850 Inc. from any liability for injuries that may occur during training sessions.

hereby grant Foundations 850 Inc., including its right to use photographs and/or videos of my child for promotional and marketing purposes, including but not limited to, the facility’s website, social media, and printed materials, both within and outside of its designated address at 516 Mountain Drive Unit 102, Destin, FL 32541.I understand that my child’s name will not be used in conjunction with these images.

In consideration of my child,

being permitted to participate in training and activities provided by Foundations 850 Inc., including activities conducted outside of its designated address at 516 Mountain Drive Unit 102, Destin, FL 32541.

hereby release and hold harmless Foundations 850 Inc., its staff, and representatives from any claims, liabilities, or expenses, including but not limited to medical expenses, that may arise as a result of my child’s participation in these activities. I understand and agree that this waiver is binding not only on myself but also on my child’s heirs, executors, and administrators.
